2009 FOOTBALL & CHEER SIGN-UP LOCATIONS


Note: All sign-ups are held in the cities of Diamond Bar, Walnut, and Phillips Ranch. 
Parents should bring a copy of their child's:
 
 
  1.  Birth certificate (original and a copy)
  2.  Proof of residence (utility bill dated from 03-01-09) 
  3.  When your child's June report card arrives, please bring two (2) copies

All football players will be weighed at the time of sign-up.
 
 
Football Cost
  • Regular registration - $220
  • Additional child - $200
  • Discounted registration - If paid by July 1, 2009, the regular registration is $200 and additional children are $180 each (save $20 per child).
  • Minimum deposit - $40
  • All registration fees must be paid by the start of the season
Cheer Cost
 
  • $100 plus Cheer Uniform

All paperwork and balances need to be turned in by August 1, 2009, or the player WILL NOT take the field, unless arrangements have been made with Lisa Rodarte.

Refunds

It is the policy of Diamond Bar Pop Warner Football Association NOT to refund registration fees once the child begins practice or takes the field.  Any request for refunds prior to the 1st day of practice must be submitted in writing to the association.  If the child is removed due to not making weight prior to certification, a pro-rated refund will be allowed once any issued gear and/or fundraiser items/money is turned in.  If DBPW administratively drops a child, all monies will be returned.  Upon board approval, monies will be mailed within 30 days.
