TAKE OUR SURVEY

Dear FCYFP players and parents:

The FCYFP Board is seeking your input on our softball program.  As former player and parent members, we hope you have found value in participating in our program and will take a few minutes to reflect on your time with us. The Board would also like to reflect on our past seasons to help improve the experiences and development of our softball players. To accomplish this, we have created a TWO PART SURVEY using Survey Monkey. The results are ANONYMOUS and will be kept CONFIDENTIAL. They will help the board to see if we are meeting the goals of the organization. We also hope the results will help to identify areas where we are succeeding and where we need to improve. Please answer both parts. Each part includes 10 rating-scale questions which should take no longer than 5 minutes of your time. Both players and parents are encouraged to take the survey. Our goal is to gauge the overall opinion of our program and thus both perspectives are important. Please note: The survey parts are not electronically tied together. So when you finish Part 1, come back to this page to get to Part 2.
