• 2023 Participant Contract/Parental Consent (Pop Warner Form)

• 2023 Scholastic Eligibility (PW Form)

• 2022-2023 Report Card

• Current proof of residence ( Utility Bill)

• Birth certificate (ONLY FOR NEW PLAYERS-original kept until certification is complete)

• 2023 Parent Manual signed (Issued at Season Kickoff BBQ)

• League Registration Requirement: Each Athlete: 1 case of Gatorade (24-20 oz. bottle case) & 1 case of soda (min. 18 count-12 oz. cans-cases) Coke/ Pepsi products, OR $50.
