Team Captain Information
*The application with the non-refundable deposit ($50) is due before the beginning of each season. You can also send in the entire team fee with application ($350).
1) The first week of play, the signed roster/waivers are to be handed/mailed in along with the remainder of your team fees ($300.00). There is a $10 per week late fee if we don't have your team fees by the end of the first week.
2) You should make copies of all the handouts (Click on the "Handouts" link). Make several copies of the score sheets (8)and a few copies of the roster/waivers to keep with you.
Whenever you have a new player, they need to sign the waiver and you send it in. This is what we use to determine if all your players are qualified for the tournament.
3) Every week you need to email your scores by Tuesday. If you fail to report your scores, your team may receive a forfeit that week (very sad thing if you happened to win your matches!).
