Coaches, please bring this form along with all info needed to the registration date(s) listed below:

Registrations will be held at Dick's Sporting Goods - St. Joseph, MO location:

* If you did not coach a PEB team in the 2016 Fall/Summer session, you must complete a background check BEFORE Aug 13th (go to PEB website and click on “Coaches Corner” on the left side…the link to perform background check is located in there towards the bottom).  It is a $12 non-refundable fee.  Once it has been completed, the system will email the results directly to us.  You don’t need to do anything else after submitting the request.

 

*  Do NOT have your players register on the PEB website individually...registering as a team takes care of each players registration.


*   Have your players pay you directly and you will provide PEB with one check.


*   Team Insurance is provided by PEB...you do not need to purchase team insurance unless your team plans to travel to outside tournaments.  PEB insurance is only good for league play only.
