The PIAA Comprehensive Initial Pre-Participation Physical Evaluation Form must be completed by a guardian and physician in order for any student to participate in practices, inter-school practices, scrimmages and/or contests at any PIAA member school in any school year. Forms must be dated after June 1 of the current school year.

If a student already competed in a sport in the same school year and previously completed the PIAA Physical Form Sections 1-6, then Section 7 of this form must be completed for any subsequent sports seasons.
