4th through 8th Grade in 2013 are eligible.
Remember to bring completed Registration & Insurance forms.
* Football registration fee: $225; $250 if jersey is needed
* Cheerleading registration fee: $100; uniform extra

Football Players:
Click on the Online forms button on the left and submit the 2013 Player Info form and the 2013 SMOW Football Insurance & Medical form
Click on the Waiver Forms button on the left, print and complete the Football Waiver Form.
Cheerleaders:
Click on the Online forms button on the left and submit the 2013 SMOW Football Insurance & Medical form
Click on the Waiver Forms button on the left, print and complete the Cheerleader Waiver Form.
Can't Make it to Registration?
To submit your registration, print and sign forms. Put forms and payment in an envelope, mark SMOW Football on the envelope and drop off at the rectory. Please remember to include:
* Completed Waiver Form - both parents must sign!
* Completed Insurance & medical Form
* Football registration fees: $225; $250 if jersey is needed
