Every player needs to fill out the Player Application form to be eligible to play.

Player Application Form - Futsal 2017/2018

Player Transfer Form - only for players who have played a game on a team and wish to transfer to another team during the seasons. Restrictions may apply. Transfers are subject to approval by the Southeast Futsal Committee.
