Background Check Form.
If you would like to become a coach or board member with TCBA, please submit a background check form online or in person at registration. All information provided is confidential and will be destroyed at the end of the year. The online submission link will take you to a secure website where you can submit your information directly to our investigative service. As always, you can still submit your forms in person. Speak to a league representative if you have any questions.
Background checks are good for the calendar year in which they were submitted. In order to receive your ID badge in time for the beginning of the season, the deadline for submitting background check forms is FEBRUARY 14, 2012. A list of approved coaches/volunteers will be posted on this page.
ALL COACHES AND VOLUNTEERS MUST SUBMIT A NEW BACKGROUND CHECK FOR THE CALENDAR YEAR 2012 (EVEN IF YOU SUBMITTED ONE IN THE FALL OF 2011).
All Managers and Coaches must submit to a background check - NO EXCEPTIONS
WE WILL BE ENFORCING THE BADGE REQUIREMENT, AND WE WILL FORCE TEAMS TO TAKE FORFEITS IF THERE ARE NO APPROVED COACHES AT GAME TIME.
