To participate in Whiteland Youth Football during the 2016 season a players physical must be dated after April 1, 2016 and on file with the league prior to the first official day of practice (July 25th or 26th 2016).

Your personal physician is required to complete and sign the IHSAA physical form. Which is located on our "Forms & Handouts" page.
