* Copy of child's BIRTH CERTIFICATE (child must be at least 5 yrs old and not over 15 by July 31st, 2009).
* Copy of child's final end of year REPORT CARD for the school year
2008-2009.
* A sports PHYSICAL is a mandatory requirement; these forms are
available @ several doctors offices. (ask for more information)
** Fees for both Football players and cheerleaders is $100.00
per child; Inquire about Family discounts at time of sign ups !!
