Fall Soccer Kindergarten Registration Form 2009
St. Bernadette CYO is changing its registration process!
The parent/guardian of every player wishing to register for Fall 2009 soccer needs to access the Archdiocesan CYO online registration website at www.washcyo.com. Those not currently in the online system (e.g., 2008-09 kindergartners or new families) must register using the system, and those already in the system must enter the Fall 2009 grade, any other updated information (e.g., e-mail address), and review and accept the Standards of Spectator Behavior.
Each parent/guardian must then print the online registration summary information form, SIGN and date the form, and provide it to the St. Bernadette CYO with the appropriate fee. (EACH CHILD'S FORM MUST HAVE A SYSTEM-PRINTED GRADE TO COMPLETE REGISTRATION. Writing the grade on the form DOES NOT complete the registration process.)
The online registration is for grades 1-8; kindergartners need to fill out a St. Bernadette form (click on link above to download).
Steps to Registering:
1. Go to www.washcyo.com
2. Click on "Online Registration/Forms" (bottom left of the page)
3. Under "Online Registration," click on "Players"
4. Complete the 3 steps to registration (indicating your interest in coaching where appropriate)
5. Print form (one for each child grade 1-8)
6. Sign form and return it to school or mail it to Jennifer Herlihy with payment
Forms/fees can be returned or mailed before May 29 to either of the following:

Fees for St. Bernadette Fall 2009 Soccer are as follows:
Grades K-2 = $30
Grades 3-4 = $55
Grades 5-8 = $80
