HOW TO REGISTER:

1) Complete the online registration form at www.eayso.org. All registrations must be done online.
2) Complete the eSign portion of your application (the system will take you through it). THIS IS MANDATORY
3) Mail the player registration forms and a check or proof of payment for each of your children.
