Please complete the following forms in order to register your child(ren). You can print the 2013 Registration Forms , Parents Code of Ethics Addendum Form and the Volunteer Buyout Form from these links or from the “Handouts” section of this site.

● NCFL Registration Form
● Physicians Statement Form (needs to be completed by your child’s doctor in order to take the field)
● Parent Code of Ethics Form and Addendum

● Player Code of Ethics Form
● Volunteer/Buyout Form

● Copy of the child’s Birth Certificate

All forms need to be completed, with the exception of the Health Form, in order for your child’s registration to be complete. All payments must be submitted at registrations in order for the player and/or cheerleader to be registered with EFL.

Registration Fees for the 2013 Season

$85.00 – First Child

$70.00 - Second Child
$60.00 - Third Child and subsequent children

After the registration dates ALL registration fees increase by $10.00/each.
