FINAL FORMS AND PHYSICALS

A reminder to all parents and students that final forms need to be completed and you must turn in a valid physical before you can participate in or tryout for a sport at Memorial.

ONLINE FORMS!!!! IMPORTANT PLEASE READ

The Mentor Public Schools will be switching to an online form format called Final Forms.  All forms will be submitted online except the physical form.  The physical form will be the only form that you will need to turn in a hard copy of.  Parents will be receiving a letter soon explaining the log in format and procedures for filling out all the forms online.

If you deleted the email or did not receive the email you can find the parent playbook for signing up for final forms in handouts.

All forms must be submitted by August 1st to participate in fall sports

Admission fee for athletic contests

The Mentor Public Schools have joined the Greater Cleveland Conference starting in the 2015-16 school year.  The league has a policy that an admission fee will be charged for all home contests.  The admission fee is $3.00 for adults and $2.00 for students.

The Memorial Athletic Dept. will be offering a student pass for $20.00.  The purchase of the student pass is good for admission to all Mentor Middle School home contests (Shore and Ridge too).  All students purchasing a pass will also receive a t-shirt.  Adults passes will be sold for $35.00

 

Student and adult passes will be sold at the main office and at all home games.

 

Both the student and adult passes will get you into all contests at Memorial, Ridge, Shore and middle school contests at Mentor HS for the entire 2015-16 School year.  The pass will be good at volleyball, football, boys and girls basketball, wrestling and track and field events.

 

**WE WILL ONLY HAVE ONE GATE OPEN AT HOME FOOTBALL GAMES.  THE GATE AT THE GYM LOBBY WILL BE CLOSED.  ALL FANS MUST ENTER THROUGH THE EAST GATE WHICH IS LOCATED AT THE BACK OF THE SCHOOL

 

****This pass is not valid at high school athletic contests and tournament hosted at Memorial, Shore, or Ridge (Boys Basketball Tournament at Ridge, Frank Godic Wrestling Tournament at Memorial and the 7th grade girls basketball tournament at Memorial).

 


CONCESSION HELP FOR FALL SPORTS

The Memorial PTA needs parents to volunteer to help at the concession stand for the Fall Season.  We are asking each parent to sign up for one 1 hour shift.

In order for football parents to watch their child play we would like football parents to help at home volleyball games which are on Tuesday or Thursday.  The same for volleyball parents, we are asking volleyball parents to help at home football games on Wednesday's.

REMINDER!!!

A REMINDER THAT ALL STUDENT-ATHLETES MUST DRESS UP ON GAME/MEET DAYS UNLESS THE COACH STATES OTHERWISE (i.e-spirit wear or game jersey). FAILURE TO DO SO CAN RESULT IN NON-PARTICIPATION IN THAT GAME/MEET!

GAMES/MEETS AT SHORE, RIDGE, MENTOR HS OR GARFIELD PARK

When traveling to Ridge, Shore, Mentor HS or Garfield Park for a game/meet there will not be a return bus back to Memorial. All student-athletes are expected to be picked up at the game/meet site. Memorial's Athletic Department offers a wide variety of athletic programs and activities for student-athletes at the 7th and 8th grade level. It is my duty and pleasure to coordinate these activities, oversee our coaching staff, and accept your input about our athletic programs.
