SOUTH RIVER LITTLE LEAGUE

2010 SR LITTLE LEAGUE SIGN UPS
Sign ups for the 2010 Little League season will be on Tuesday, November 10th and Thursday November 12th, from 6:30pm - 8pm. They will be held at the entrance to the SR High School Gym. Players must be born between May 1st 1997 and April 30th 2001.
** Please bring the following Forms(3) and the required player/parent/guardian information requested below** The forms are available to print/fill out ahead of time, if you like (see links below).
We will have forms on hand and available at the Registration also.
-Medical info to fill out Medical Release Form (Insurance Company, Doctor's name and phone number)
-Copy of Birth Certificate (whether returning player or new player)
-Proof of Residence - 3 needed (copy of Report Card, copy of Utility Bill, and copy of Driver License)
-$85 Registration Fee ($135 family of 2). Make checks payable to: South River Little League
-$100 Work Bond Check ($150 family of 2). Make checks payable to: South River Little League
