Step 2-residency and birth certificate verification
To verify your birth and residency information please arrive with your paperwork from the online registration at the:
Eastlake Middle School Library: 900 Duncan Ranch Road Chula Vista CA 91914 on one of the following dates:
June 20, 6-8 pm
June 23, 2-4 pm
June 30, 2-4 pm
WHAT TO BRING TO VERIFICATION DATES:
All completed documents from on-line registration
· 3 copies of the Player Contract
· Signed Refund Policy
· Signed Parent Code of Conduct
üProof of Age/Birth (ORIGINAL Birth Certificate PLUS 2 copies, kept by Association)
üCurrent Physical, must be no more than one year old
üCurrent Report Card, last report card issued by the school, this can be submitted at a later date.
ü2 Types Proof of Residency (current utility bill, mortgage statement, homeowners association bill, etc.), 1 copy of each to be kept by the Association

Flag Player: $260
Tiny Mite: $315
Tackle Player: $315
Cheerleader: $220
Computers and support staff will be available on site at all of the verification dates if you need assistance in completing the on-line registration process. Computers are limited so please try your best to complete the on-line registration at home before residency verification.
