Attn All coaches and Asst coaches
Due to a new rule that Cal North/CYSA is implementing for the 2013 season and forward to meet insurance requirements, we will need all coaches and asst coaches to complete online and submit this new CYSA 1650 form. This new form is required before a coach pass will be printed.

What to have for your child's registration:
*complete packet should include fee of '$155.00 per player', a signed registration form, copy of your child's birth certificate, recent picture, signed ethics agreement, uniform size sheet, and a special request form if you are requesting anything*
