2012 FOOTBALL FORMS AND FEES
All required football signup forms have been uploaded under the "Handouts" section.
The following forms are required:
2012 Participant Contract and Parental Consent Form
2012 Player Physical Form
Certified Copy of Birth Certificate
Final Report Card from school year 2011-2012 (when issued after school year ends)
Forms should be completed and turned in on date of signup.
Additional copies of all forms will be available at signup location.

Individuals interested in volunteering should complete and turn in the following forms:
2012 Volunteer Application
HYL Background Check and Safety Policy

2012 Football Registration Fee: $128.00
($125.00 HYL Fee and $3.00 County Park Usage Fee per Participant)
