What Your Team Need BEFORE Registration
$500.00 per team PAID IN FULL, Team Contract, Copy of Field Insurance, Team Roster in ABC order by last name, Copy or Receipt of Child Abuse Clearance, Permission/Waiver Form.
If you are moving your Seniors up to the Showcase Division, you will need a Parental Permission Form for each player.
What Your Players Need BEFORE Registration
(2) forms of ID; Report Card, School ID and/or State ID, Copy of Birth Certificate, Player Waiver Form.
