Medical Forms: Two Forms – 1) Parent complete and sign Medical History. 2) Physician complete Section 2. Must be filled out and stamped by your doctor and handed in prior to first practice in August. Physical date must be on or before August 1st. NO FAXED COPIES WILL BE ACCEPTED. MUST BE ORIGINAL.
Work Bond:: Please return postdated check for $100.00 with registration.

CHEER PROGRAM ALSO REQUIRES:Spirit Squad-Rules and Regulations for All Cheerleaders: Please read this carefully. It must be signed by both the cheerleader and parent/guardian and sent back along with registration at sign ups or by June 1st.

All our coaches are volunteers.They give a great deal of themselves to provide a quality experience for your son(s)/daughter(s). Their goal is to teach not only the skills, but also sportsmanship and leadership. Lets work together and make this season enjoyable for all involved.
