Sign Ups for 2012 LVSD Pop Warner Football, Cheer and Dance
All Participants must bring:
Original Birth Certificate- copies will not be accepted* New participants to LVSD*
Proof of Residency- Utility Bill: Gas, Water and Electric bills are the only acceptable bills
2011/2012 FULL Year Report card (to be turned in June 2012)
Football/Cheer participant must be present at the time of sign up
**Fees will only be accepted in FULL or Half, no Exceptions**
Regular registration fees are $225.00 for both Football & Cheer
LVSD Offers sibiling discounts and payment arraignments
LVSD Accepts Cash, Checks, MasterCard, Visa, Discover & American Express on site There is a $5.00 associated with each transaction.

10am -2pm Classroom- 905
This is also our Physical's date
we will have Physican's available for your 2012 required physical
*$20.00 cash only for physicals*
all physicals must be the Pop Warner physical form
