know that running a road race is a potentially hazardous activity. I should not enter and run unless I am
medically able and properly trained to cmplete the course. I agree |
| to
abide by any decision of race official relative to my ability to safely
complete the run. I assume all risks
associated with running in the 2012 Santa Chase Half Marathon or |
| 5K
run including but not limited to falls, contact with other participants, the
effects of weather, traffic and conditions of the road, all such risks being
known and |
| appreciated
by me. In consideration of acceptance
of this entry, I hereby , for myself and my heirs, executors and
administrators, waive any and all rights, claims and |
| damages
I may have against the sponsors, coordination groups, Mattoon Youth
Wrestling, City of Mattoon, Illinois and any individuals associated with the
Mattoon |
| Santa
Chase Half Marathon and 5k Run. I
acknowledge and agree that the official race director reserves the right to
delay or cancel the Event if the director believes the |
| conditions
on the race day are unsafe. In the
event the race is delayed or cancelled for any reason (including but not
limited to emergency or local or national disaster, |
| acts
of God or the elements (including without limitation, rain, snow, sleet,
hail) or any other cause beyond the control of the Mattoon Santa Chase Event
there shall be |
| no
refund. I acknowledge that all entry
fees paid or non-refundable and non-transferable. |

| I
hereby grant permission to the parties listed herein to use my name and
photographs, videotapes, motion pictures, recordings or any other record of
my participation in |

event for any purpose. Lastly, I
understand that pacers, strollers, bicycles, roller blades/skates, or pets
