Parents please remember that all required documents MUST be received before your child's first practice. These documents are listed below. This is a National Pop Warner requirement and therefore there are NO EXCEPTIONS. The following documents MUST be completed and submitted before any child--football or cheer--takes the field or picks up equipment. If you have questions about these documents, please contact your coach.

2014 Parent Player Consent Form--front and back, signed by both parent & participant.
2014 Player Physical--front to be completed and signed by parent, back to be completed and signed by doctor
Additional documents needed:
Copy of birth certificate, please make copy for association to retain
Copy of FINAL 2014 report card that includes ALL four quarters, please make copy for association to retain
Wallet size color photo with participants name on back, will be used in the team book and can not be returned
If you do not have all of these documents, please come out today to turn in as many as you have.
Your child may watch practice until all of the documents are turned in.
