Registration Requirements:
1. Completed Contract
2. Completed Physical must be on Pop Warner 2012 Form No Exceptions!!!
3. Original Birth Certificate for authenticity as well as a COPY at the time of Registration
4. Copy of Current Month's Utility Bill ( Only forms accepted Water, Gas or Electric Bill) No Exceptions!!!
5. 2 copies of 2012 Full years Final report card must have all semesters for Jr, High/ High School and all trimesters for Elementary Due Aug 1st, 2012
6. Registration Fee Early Bird $220.00 until May 27th
$250.00 May 28th until Aug 1st
7. Football Players must be present at time of sign-ups to be weighed. **Must**
All registration requirements must be met prior to a child being placed on a team. Failure to provide all required documents and registration fees in full by July 28th would result in your child losing their place and not taking the practice field.

**Rental of Football Equipment (Helmet, Shoulder pads, game and practice uniform)
**Socks, Picture Package, Away Jersey and Trophy at End of Season
Parents are responsible for Girdle, Cup and Rubber Cleats
