What is required?
*Official county issued birth certificate-MUST be ORIGINAL!!
[Will be kept by SBPW until certified roster is complete]
*Player Contract on 2013 Pop Warner Offical Form-under forms tab
*Physical on 2013 Pop Warner Official Form-under forms tab ** Must be completed by physican** physical must be done in 2013, 2012 physicals will not be accepted
*Copy of Current Utility Bill (water, electric or gas bill)
*Copy of Full years report card due in June
*Child is needed at sign-ups for weigh-in
*Fee $250 cash or Money Orders only.. No Personal Checks Allowed!! In order to guarantee a place on a team you need to be paid in full or have 1/2 of payment down at time of registration
