The difference between a successful person and others is not a lack of strength, not a lack of knowledge, but rather in a lack of will.

then will you be a success.

FHSAA Pre-Participation Physicals are REQUIRED prior to athletes practicing. You will be required to have this BEFORE you can practice. THERE ARE NO EXCEPTIONS. Read the message below completely. Additionally all athletes must have the FHSAA consent and liability signed.
Both the FHSAA Pre-Participation Physical and the FHSAA Consent must be completed and recorded by the school and your eligibility verified before you can be part of the team. Both forms are attached here and can also be found on The website www.SMALacrosse.com or www.FHSAA.org Jim Villotti MD and Gene Erquiaga MD will do the SMA lacrosse pre-participation physicals on 1/06/2011 ( Friday ) beginning 1 PM ( if possible ). The Physicals will be performed on SMA school grounds.
The students MUST have their history/ consent / athletic form completely filled out and signed by their legal guardian or they will not have their physical exam.
We will require non-coach volunteers ( 2 ) to assist the doctors and observe the process. I must have notice of these 2 volunteers now. This is a physician and school requirement. If you can assist in this capacity please notify us now. If we do not have we will be unable to conduct this service.
Additionally the Dr's can and will perform for the female athletes but we will need two mom's to assist and observe the process
Athletes should be prepared to discuss :
Medical history Allergy history( especially asthma) Trauma history ( injury ) Hospitalizations and surgeries. Medications Concussion history Tetanus shot
Students should be prepared to have their groin area examined for hernias and/or testicular abnormalities.
Helmets, eye-wear, joints supports, mouth pieces and braces should accompany the kids to the exam. Parents are welcome to attend their children's exam.
The whole purpose of PRE-PARTICIPATION exams is for the safety and well being of children participants. Students should be encouraged to have their physicals completed by their own personal physicians. Athletes who get their check-up with their own doctors need not see us for participation approval. This is in step with recommendations from AAP, ACSM, and FAFP. Coaches, athletes, and parents should understand that the physical is an imperfect attempt to identify potential problems that may result in the need for further evaluation before participation EVEN in practices. Some examinees WILL BE UNABLE to participate until these identified issues are cleared up. FHSAA recommends restricting students who are fully sexually developed from participating in VARSITY contact sports ( this includes lacrosse ). Athletes and their parents should be made aware that any student who has not yet experienced Tanner post pubertal changes ( axillary hair - etc ) will not be cleared medically for VARSITY play.
Varsity athletes in particular should be expected to be in good physical condition before participation. Fit students who are in shape, rested, well hydrated and nourished, practice good personal hygiene, are motivated and respectful of themselves, the game, their coaches, the rules, teammates, and opponents experience less injury.
The exam form is not transferable for other sports or uses.
Ask the students to bring a beach towel as a cover-up. If the kids have information or letters of clarification from other health care providers please ask them to bing a copy of same for team records.
We insist that unsafe and risky off field behavior be cause for team dismissal. Illicit drug use, alcohol use, and cigarette smoking will preclude participation.
The Dr's are donating their time to benefit the SMA Lacrosse program a nominal donation of $25 is suggested for this service. Checks can be made payable to SMA and will be collected at that time.
