Our first signups are this weekend, April 28th at the Kenny Dixon Sports Complex, 9-1pm. Check the handouts tab for all information and don't forget, fan shirts will be available for purchase starting this weekend!

Copy of Birth Certificate Final report card Physical and Medical History form (2nd page to be completed by Physician) Participant and parental consent form Wolfpack Registration
Optional: Pre-registration Form (if you are paying in installments you MUST fill out this form, If you pay in full, you need not fill this out but we recommend everyone read it.)
Volunteer Application (if you are interested in assisting in coaching or being a team/squad mom, you must fill out this form)
Pre orders for fan and parent shirts will begin soon! You will be able to claim yours early this year so keep an eye out for more information, coming soon.

Welcome to the 2012 South Sumter Pop Warner season! We are preparing for the upcoming season early this year and would like to give you and your child as many options as possible to enable them to become involved in our program. Below, you will find a breakdown of the costs involved in signing your child up and what the fee entails, as well as information on our pre-registration program that will begin this season. Please bear in mind that while your child may not receive new equipment each year, all equipment must be reconditioned at regular intervals to ensure the child’s safety.
If your child is participating in our football program, your sign-up fee assists in: Uniform jerseys, pants, weigh in attire, socks, mouthpieces and chinstraps, reconditioning of helmets and shoulder pads, league fees due to Pop Warner, per child costs to the City of Bushnell, League insurance and operating costs.
If your child is participating in our Cheer or Dance program, your sign-up fee assists in: Uniform tops and skirts, bloomers, bags, shoes, accessories such as bows, league fees to Pop Warner, per child costs to City of Bushnell, league insurance and operating costs.
Cost per child is $150.00 each. For the THIRD CHILD OR MORE PER HOUSEHOLD, The registration fee is $125.00. For example: Three children would cost $425.00; $150.00 each for the first two children, $125.00 for the third.
This season you will be given the opportunity to begin paying for your child’s registration now. You may visit our website, www.leaguelineup.com/sswp and select the “handouts” tab to print all the necessary forms in advance.
JULY 14TH, 2012, IS THE LAST DAY FOR ALL PAPERWORK AND PAYMENTS TO BE SUBMITTED OR YOUR CHILD WILL BE INELIGABLE TO PARTICIPATE. Unfortunately, there will be no exceptions. Thank you for your understanding and cooperation in this matter.
