REQURED PAPERWORK
Paperwork I must have by Friday, September 12, the following must be turned in for your child to be rostered:
U.S.A. Hockey Registration (IMR)
Birth Certificates
Background check (Needed only for fall season)
Parent Acknowlegement Form (Needed only for fall season)
Coaches Code of Ethics (Coaches only)
Copy of Level 3 Coaches Card (Coaches only in the fall season)
The following forms are not mandatory for rostering but must be filled out and returned to me ASAP for credentials review in early January:

ICE FEES FOR THE 2010/2011 SEASON
The ice bill will be $150 per and will be due by the 10th of every month.
*Because the THA is making our March bill due on March 1, 2011 our last ice bill of the season will be due on Sunday February 21, 2011 in order to make the due date*

QUARTER PER GOAL
We will be doing a quarter jar this year for goals. For every goal our team scores please put a quarter in the jar. For every shut out our goalie gets we ask that you donate $1 to the jar. This is a quick and easy fundraiser for the team.
