*Completed 2010 Tri-Cities Registration Form(see Handouts)
*2010 Pop Warner Participation form(see Handouts)
*Copy of Birth Certificate
*3"x5"Picture of Participant,"no hats or sunglasses"
*Physical,"must be done on 2010 Pop Warner physical form(see Handouts)
*2009/2010 full year report card.
*Registration Fee:
Early Bird: $175.00 before 5/4/2010
$200.00 after 5/4/2010
