2016 Season Check List
Here’s a convenient checklist for you to get you ready for the 2016 season.
STEP 1: REGISTER FOR OUR 2016 FOOTBALL AND CHEERLEADING SEASON if you haven't already done so
Beat the clock and take advantage of our early bird registration discounts as they will expire on June
18. Note that anyone participating in the early bird discount can still take advantage of our payment
plan, but must do so by! We also have great sibling discounts available (Note: Flag football and
Mitey Mite participants do not qualify for sibling discount consideration due to their already deeply
discounted registration prices) .
STEP 2: COLLECT REQUIRED DOCUMENTS AND FILL OUT OUR ANNUAL PAPERWORK PACKET
Birth Certificate – If you are new to our program, you must turn in both a certified (raised seal) copy of
your son or daughter’s birth certificate, and, a photocopy of the same. After Central Mass certifies our
team books in late August, we will return the certified birth certificate to you.
Report Card – When your son or daughter comes home on their last day of school with their full year
(2015/2016) report card, please put it aside in a safe place. We will need a photocopy of this report card
for our books. Academic performance measurement is a part of our book certification process and an
important focus in our programs. (Note: Those coming in from Pre-K may not have a report card – this is
o.k. if one doesn’t exist).
Medical Clearance – This is very important and can be found as part of our paperwork packet. Your son
or daughter’s primary care provider must sign the form in the participant paperwork packet in order to
indicate that they are medically cleared to participate in our programs (no physical forms are accepted).
The document must be signed and dated in 2016. This does not necessarily mean that you need to
have a physical this calendar year, but each provider has different requirements. Please note that your
child will not be able to begin practices until this medical clearance is obtained – there are absolutely no
exceptions to this rule. We encourage you to reach out to your son or daughter’s provider ASAP to get
this paper signed. (Mid-level provider signatures (NP’s or PA’s) are acceptable).
STEP 3: MARK YOUR CALENDARS FOR THE START OF PRACTICE
Tackle football and cheerleading practices will start on or about Confirmation will come from .

NOTE: If you have vacation planned in August, simply reach out to your head coach as the season start
approaches. Note that all tackle football and cheerleading participants must have a minimum of 10
hours of conditioning prior to participating in the Jamboree.
STEP 4: BECOME A VOLUNTEER
We are still seeking the following help:
- Coach volunteers (Flag and Tackle Football Teams)
- Team moms (Flag and Tackle Football Teams)
- Assistant Equipment Coordinator
- Apparel Coordinator
Absolutely no experience is required and we will train and support you all of the way!Remember, most
all of the adults participating in the league are parent volunteers just like you. The only way our
programs are successful are through the generous donation of time and support from our parents.

STEP 5: STAY IN TOUCH
We will communicate important information frequently throughout the season via several mediums
VERY IMPORTANT: Make sure that all of your contact information is accurate and up-to- date in each of
the registration systems (email addresses, phone numbers, home address, etc.).
