MEDICAL RELEASE
As parent/legal guardian of the above player, I hereby give consent for any emergency medial treatment as approved by his/her manager, coach or any other adult escort in case of illness or injury while participating in league activities such as practice, travel, league or tournament play during the baseball season. I also hereby give my consent and approval to his/her participation in any/all Bethlehem Mickey Mantle, Inc. (BMM) - BRAVES Travel Baseball Club activities. I assume all risks and hazards incidental to such participation in any/all activities and hereby waive, release and agree to hold harmless BMM, its Board of Directors, manager, coaches and volunteer participants, from any claim arising our of any injury to the above player.

Players Fee - 2016 Summer Season (TBD)

The BRAVES baseball program includes the loan of BRAVES uniform jersey top (a separate $100 uniform jersey deposit may be required) and a new BRAVES Hat for each player (you keep).
The actual baseball game schedule begins around Memorial Day Weekend and runs thru the end of July, with post season tournament opportunities.
It is advised that players DO NOT consider taking a vacation until after our season ends the first week of August.
