All paperwork includes: coach's info. sheet, roster sheet, uniform order form, and coach's schedule request form.
Failure to submit any of the following will result in no available practice fields.
Failing to submit the uniform order sheet by the due date will cause delay in your uniform shipment - causing you to start games without proper uniform and potential extra uniform fees.
Failing to submit the schedule request form by the due date will cause interference with your schedule.
WE WILL NOT BE ACCEPTING ANY SCHEDULE REQUESTS PAST THE DUE DATE: THURSDAY, FEBRUARY 1ST. IF YOU ARE UNABLE TO PASS BY THE OFFICE, PLEASE FEEL FREE TO EMAIL IT TO US AT: firstname.lastname@example.org
