TMYBA Sponsor Form (2018 Season)

TMYBA Sponsorship Levels:

Single - 1 team - $300
Double - 2 teams - $500
Triple - 3 teams - $750
Home Run - 5 teams - $1000


All sponsors will have their company name or logo on team uniforms. Logo must be received by March 15 prior to the season.

Most sponsors prefer to sponsor teams of specific children. If so, please fill in the following information. If this area is left blank, we will assume your sponsorship can be assigned to any team. Some sponsor more teams than children. For example, you might elect to be a Double Sponsor for $500, but only have one child to sponsor. If this is the case, fill in the one child and we will assume the second sponsorship can be assigned to any team.

I need to know if you prefer to have name or logo on the uniform. If you have a complicated logo, name is more visible. Please use the drop down menus below to indicate if returning sponsor and if new sponsor, do you prefer name or logo. Any new logos need to be emailed to me by March 15.
You must select the appropriate sponsorship level below and then click "Submit Form." At this point, next page will allow you to "Checkout." When you select "Checkout," you will be redirected to PayPal's site to pay by credit card or PayPal account. You do not need a PayPal account to use the site.

If you prefer to pay by check, exit the PayPal site and write check payable to TMYBA and put Sponsor's name on Memo line if not a company check. Mail to:

At times, we have some leagues with more sponsors than teams. If your child's league is full, I will contact you. You will have the choice of a refund or sponsoring a team in another league.
