After completing the online registration, please go to Registration Forms on the TMYBA website. A signed Medical Release, TMYBA Release of Liability Agreement and Parent & Athlete Concussion Agreement will need to be provided to coaches before participating in any Heat practice or game.

By clicking "submit form" below, thus completing this registration form, you release and discharge the Thiensville Mequon Youth Baseball Association (TMYBA), Heat Program, umpires, volunteers, and directors from all action, suits and demands whatsoever in law or in equity, including but not limited to, the risk of personal injury from participating in practices, games and tournaments and the risk of loss of personal property by theft or otherwise.
