2017/2018 Trail Appliances Team Registration Form

I would like to submit an application in the RIC Hockey 1-4 Tournament. I understand that this application does not constitute automatic acceptance into the tournament. Teams will be notified of acceptance by 30 November 2017. If not accepted into the tournament, entry fees submitted with the application will not be cashed and all money order/cashiers cheques will be promptly returned.

I understand that if multiple teams from the same association apply to the tournament, team selection preference will be given based upon the date the application is received. Please note, it is possible that more than one team from the same association could be accepted into the tournament.

If for any reason the team is unable to attend after acceptance, I will notify the RIC Tournament Registrar immediately. Teams canceling within 21 days of the start of the tournament may be subject to forfeit of their entry fee, depending upon whether another team can be found as a replacement.

By submitting this registration form:
a) the Team Manager and Head Coach agree to abide by all the tournament rules and rugulations; and
b) the Team Manager and Head Coach release the RIC Initiation Tournament, Richmond Minor Hockey Association and Seafair Minor Hockey Association, all officials and volunteers associated with the Tournament, from any liability for injury or accident whic hmay be incurred by any player or team officials while attending or traveling to, or from the RIC Initiation Tournament.
