I the player ot the parent/guardian of the above mentioned minor child/player, do give my approval to their participation in Ohio Warriors Baseball for the 2013 season. I assume all risks and hazards incidental to such participation and we do hereby waive, release, absolve indemnify, and agree to hold harmless Ohio Warriors Baseball, the organizers, sponsors, and participants of the programs activities for any claim arising out of an injury to our child to the extent covered by accident or liabililty insurance. I also authorize the representatives of the Ohio Warriors to seek appropriate medical attention if necessary in the event that an injury occurs and I am not present.
