WAIVER INFORMATION

My son and or daughter understands, and is willingly participating in AG Sports and Training basketball sessions under the conditions that follow:
I, the undersigned, hereby certify that I am the parent or legal guardian of the basketball participant. AG Sports and Training basketball sessions will include basketball performance drills, game-like scenarios, and physical conditioning . Common injuries that might occur include sprains and strains of the following: ankles, knees, and fingers. I hereby give permission to the staff of AG Sports and Training to seek appropriate medical attention for the participant if necessary. I will be responsible for any and all costs of medical attention and treatment. I, the undersigned, waive, release and forever discharge AG Sports and Training and its staff, employees, and representatives from any and all liability, claims, demands, actions, and causes of actions whatsoever arising out of or related to any loss, personal injury, or property damage that may be sustained or occur during participation in AG Sports and Training basketball sessions .

TERMS AND CONDITIONS

4-session and 8-session passes begin the first date of use. After the first day of use, the pass purchase is FINAL, NON-REFUNDABLE, NON-EXCHANGEABLE. SORRY NO EXCEPTIONS. All scheduled sessions are subject to change. If sessions are cancelled due to insufficient registration or inclement weather, registrants will receive a make-up session.
Please check the website for weather updates, cancellations and/or scheduled make-up dates.
WALK-INS are WELCOME, but there is NO GUARANTEE that a session will be available. Sessions may be cancelled in the event of insufficient registration. We suggest you SCHEDULE your sessions in advance to ensure a session is held. Again, ALWAYS check the website for updates.
If you SIGN-UP for a 4-week or 8-week session pass and you wish to cancel the entire pass PRIOR to your first date of use, you must cancel within 3 days of purchase for a refund minus a $20 cancellation fee. You MUST contact us by Email or in Person with written confirmation.
There are no refunds for purchases 4 days or older.
