LIABILITY WAIVER / CONSENT TO PLAY
I acknowledge that there are risks involved in playing hockey, and in consideration of my child/ward participation with the Bancroft Jets hockey Association, I hereby absolve the Bancroft Jets Girls Hockey Association from any and all liability arising from participation in any activity directly or indirectly associated with the previously mentioned hockey association occurring on or off of the ice. My child has permission to participate in the Bancroft Jets Girls Hockey Association activities and i give the Bancroft Jets Hockey Association permission to render medical attention to my child/ward, should it be necessary. It is further agreed that all rules and regulations will be followed by the participant and / or parent / guardian.
