Please complete the following information to be a Sponsor for East Bay Youth Athletics.
Each Sponsor will get recognition on league website as well as acknowledgment opening day.
Please retain a copy of this form for your tax records.

For Banners: For Color Banners, please provide a digital copy of your logo, business card or ad. The Best format to submit is EPS or high resolution JPEG (300 DPI or greater)

World Series (Team): $300 - Company name on jersey, recognition on website & team plaque.

World Series (Banner): $300 - Banner on field of choice for 1 year & recognition on website.

Grand Slam: $500 - Company name on jersey, banner on field of choice for 1 year, recognition on website & team plaque.

Field of Dreams: $1,400 - Company name on jersey (2 spring/2 fall),
2 banners on fields of choice for 1 year, recognition on website & team plaques.

Scoreboard sponsorship: $3,500 - Name/Logo on scoreboard for 5 years, logo on website, company name on 10 team jerseys (5 spring/5 fall) for the first year, 2 banners on fields of choice for 5 years & team plaques. (SOLD OUT UNTIL SPRING 2019)

Sponsor a Field: $6,000 - Name/Logo on banner for 2 years, logo on website, company name on 4 team jerseys (2 fall/2 spring) for the first year & team plaques.
