WAIVER INFORMATION

As parent or guardian of the above named participant(s) in the Graham Youth Athletic Association baseball or softball program, I hereby give my approval for my child’s/children’s participation in his/her chosen baseball or softball league. I hereby waive, release, absolve, indemnify and agree to hold harmless the Graham Youth Athletic Association, organizers, sponsors, supervisors, participants and persons transporting my child to and from activities from claims arising from injury to my child except to the extent covered by accident and liability insurance.

I will furnish a certified birth certificate for the participant named above upon request of the Graham Youth Athletic Association Board in a timely manner.

I recognize that I will be required to contribute additionally to the Graham Youth Athletic Association as follows:
1.) Assist in maintaining the ball fields by picking up trash left by my family or myself.
2.) Provide transportation for my child to and from practices and games.
3.) I will show sportsmanship and respect to others during all activities. I will set a positive example for all children to follow.
4.) I will be held liable for damage to GYAA property by my unattended children. Any serious violations of the items listed above may result in being asked to leave the park and possibly face further actions depending on the severity of the offense.
