Inland Empire Premier Baseball League Participant Waiver
In consideration of being allowed to participate in any tryout, league practice, league game, or event organized by the Inland Empire Premier Baseball League or any Inland Empire Premier Baseball League individual or entity to include, but not limited to Inland Empire Premier participating venues, parks, and or schools, collectively the “Baseball Parties,” the undersigned prospective player or manager agrees to assume all risks incidental to such participation (including, without limitation, injury to loss of person or property).

BY CHECKING YOU AS THE TEAM REPRESENTATIVE AGREE, hereby release and forever discharge each of the following baseball parties from any and all liabilities, claims, actions, damages, costs or expenses of any nature whatsoever, whether in law or equity, known or unknown, that any of the undersigned ever had, now has or hereafter can, shall or may have against any of the Baseball Parties arising out of or in any way related directly or indirectly to players participation in such league.

Each of the players, parent(s) or guardian(s) of such player hereby agrees to indemnify and hold harmless each of the Baseball Parties from and against any and all liability, claims, actions, damages, costs or expenses of any nature whatsoever, whether in law or equity, known or unknown, incurred by any of the Baseball Parties and arising out of or in any way related, directly or indirectly, to players participation in the Inland Empire Premier Baseball League, workout, tryout, or other activity. Each of the undersigned player, manger, parent(s) or guardian(s) of player hereby ratifies player’s execution of this waiver and release form.

At least one player’s parent or guardian signature is required in order for a player under the age of 18 to participate in the Inland Empire Premier Baseball League. Players over 18 must still sign the waiver to participate. The manager is responsible to ensure that every player on his team has signed and dated this form PRIOR TO PARTICIPATING in an Inland Empire Premier Baseball League event. Player may not play until signed. The manager will be personally liable for not having a waiver for each player on his team. The league is free from liability against persons who do not sign the waiver and play or participate in any league event under the direction of such manager or player acting as a team representative or manager. By signing below you agree and understand the terms of this waiver do not expire as long as you participate in the Inland Empire Premier Baseball League.

BY AGREEING YOU ARE RESPONSIBLE FOR THE ABOVE LISTED INFORMATION FOR ALL OF YOUR PLAYERS, AND RESPONSIBLE FOR RELAYING THE INFORMATION TO YOUR PLAYERS/TEAM/ AND SPECTATORS.
