WAIVER INFORMATION
Terms of Registration

I, the legal guardian of the above named player and the named player both understand that risks of injury exist in athletic competition and I/we will not, in any way, hold CAVALIERS Youth Foundation Inc or any of its subsidiaries, representatives of the organization, or ownership/management of facilities that we use liable or responsible for any injury that I may incur while participating in events associated with CAVALIERS Youth Foundation Inc or any of its subsidiaries or affiliates.

In event of an emergency, I/we give any member of CAVALIERS Youth Foundation Inc the authorization to release and/or discuss any information regarding my case to any responding medical personnel.

I/we have provided health insurance information to CAVALIERS Youth Foundation Inc. and agree to consult that insurance provider for medical costs/coverage in the event of an injury and in no way will I/we hold CAVALIERS Foundation Inc or any of its subsidiaries or affiliates liable for any medical costs.

Parents, in the event that your child is under the age of 18, your acceptance of the terms of this agreement will be considered an acceptance on your child's part as well and it is required that you ensure that your child understands the rules listed in the Player's Agreement.
I will adhere to all of the stipulations in the Player's Agreement:
*Respect all figures of authority while under the direction of CAVALIERS Youth Foundation Inc.
*Represent CAVALIERS Youth Foundation Inc in a positive manner while under the direction of CAVALIERS Youth Foundation Inc.
*Demonstrate self control, temperance, and maturity especially when frustrated, angered, or disappointed.
*Consult the chain of command to resolve issues pertaining to CAVALIERS Youth Foundation Inc.
*Acknowledge and accept disciplinary actions that may be imposed upon a player for rule infractions
*If released, I/we am/are no longer a member of CAVALIERS Youth Foundation Inc and am in no way entitled to benefits or assets of the organization.
*Players must always seek permission to separate from the team for any reason while under the supervision of CAVALIERS Youth Foundation Inc.
*There must be contact with a coach 24hrs before an event and no less than 2 hrs before a practice if an absence or tardy is expected.

Terms of the Parent's Agreement
1. The Chain of Command for Cavaliers Youth Foundation, Inc applies to parents as well as the participants. Although you are an adult, you are responsible for maintaining an optimal level of respect for the Chain of Command (ie officers of Cavaliers Youth Foundation, Inc). We do understand that, in the heat of competition, emotions are sometimes escalated. We ask that you, as a parent and adult, demonstrate positive self control and privately discuss any concerns with Cavaliers Youth Foundation Inc staff.
2. No parent is permitted to engage into any verbal or physical altercations with another parent/adult/fan while attending a Cavaliers event.
3. Betting or waging on the outcome of a game, tournament, or player’s production will result in immediate dismissal from the organization.
4. Belittling of a player will not be tolerated on any level and officers of Cavaliers Youth Foundation, Inc reserve the right to determine what is considered belittling. You will be dismissed immediately if you are guilty of this type of child abuse.
5. Adults communicate differences amongst one another. Spreading heresay, gossip, personal opinions regarding players or coaches, publicly expressed negative comments, taunting, or any behavior considered damaging to the unity of the team will not be tolerated and will result in corrective coaching.
6. Your child isn’t owed anything! This includes anything tangible or intangible and especially playing time. Please do not allow yourself to believe that registration fees rent playing time. In the past, parents have been disgruntled and vocal about their daughter's lack of playing time or another player's abundance of playing time. Lady Cavaliers IS NOT a recreation/community team but is a college prep/exposure team. It is a "competition" environment where playing time is TOTALLY based on your daughter's practice performance evaluations and is determined by the coaching staff. Once again, registration fees DO NOT guarantee playing time.
7. Donations, fees, dues, or pledges are NON-REFUNDABLE. Understand that they will be used for Cavaliers Youth Empowerment Inc. purposes including any expense or reimbursement of an expense incurred relative to inducing the optimal level of operation for the organization. Also, if you decide to leave the organization or if you are dismissed from the organization, you are not entitled to any type of compensation.
8. Your child is responsible for being in practice and will be penalized at the discretion of the coaches for poor attendance. Players will also be excused at the discretion of the coaches and any decision made regarding a player’s absence will not be debated.
9. You are not bound to this organization. This is not a recreational team. Our objectives have been established and will not be compromised. If you do not agree with our methods of accomplishing our objectives, we are open to suggestions but are not obligated to make changes based on those suggestions. In the event of irreconcilable differences, we simply suggest that you remove your child and play elsewhere.
10. Going forward, our organization demands that every person behaves such that our standards are never questioned or compromised. If you cannot adhere to this, we ask that you leave now or be dismissed later. We WILL NOT tolerate less.

Corrective Coaching includes: Banishment from practices, banishment from all Cavalier activities, fines imposed, child suspensions, and child dismissal.


In summary, registration fees are non-refundable and in no way pay for playing time. Playing time must be earned and will be determined by the coach. Also, I understand that registration fees, donations, sponsorships, pledges, and fundraiser monies will be used for Cavaliers Youth Foundation Inc purposes including any expenses or reimbursement for expenses incurred relative to the optimal operation of the organization.

I understand that the uniform issued to me/my child is a loan and is the property of CAVALIERS Youth Foundation Inc. I will ensure that the uniform top and bottom and other provided athletic wear is returned without damage. I also agree that I am liable for loss and/or damage to the uniform set and provided athletic wear and will pay the liable amount of $105 for the uniform set and, if applicable, the value of additional provided athletic wear (shooting shirt, warm ups, etc.) within 21 days of separation from Cavaliers Youth Foundation Inc regardless of if the separation was by voluntary or involuntary means.

Other financial obligations may become necessary as events approach depending upon fundraising success. We will hold to our committment of providing the BEST exposure at the lowest cost, however, additional financial assistance may be necessary and will be the responsibility of each individual player to fulfill her due.
