LPALL Team Staff Application/Contract


I do hereby agree to read, understand, and abide by the regulations as set forth by Lincoln Park American Little League Inc. Little League Baseball / Softball Inc and Governing parties, including the following:
1. The Philosophy of Lincoln Park American Little League Inc and Little League Baseball/Softball Inc
2. The Playing Rules in their entirety as set forth by Lincoln Park American Little League and Little League Baseball/Softball Inc

Contracts submitted which include "yes" answers to question above, which are relative to activities outside of Lincoln Park American Little League (LPALL), Must be presented to LPALL Board prior to first practice, game, or any participation by applicant as a member of any coaching staff.

Manager, Coach and Team Parent Contract
Requirements:
Attend Manditory Meetings, including but not limited to 1) First Aid/Safety Awareness 2) Rules and Regulation 3) Field Maintenance Awareness
Maintain League Equipment and Respect for All, Including but not limited to:
1) Equipment Bag
2) Helmets(with safety mask, if assembled)
3) Catchers Equipment(Mask(s), Helmet(s), Throat Guard(s), Shin Guards, Chest Protector, Catchers Glove
4) Baseball/Softball Bat(s) if issued of available from general use area (building)
5) Training Equipment if issued of available from general use area (buildings)
6) Field Bases (including Home Plate-if removable)
7) Field of Play
8) Grounds Keeping Equipment
9) Positive representation of Little League and all of the Affiliations.
10.) Be a positive Roll Model and Mentor. Refrain from speaking, acting out or using social media as a negative means of representation.

Field Maintainence (grooming) will be done after each practice, game or event to assure readiness for next practice, game or event.
Repair area of wear before leaving grounds (Pitchers Mound, Batters Boxes, Catchers Box, and Infield Bases) assure game ready.
Maintain Clean/Orderly Dugouts.Empty trash cans after every practice, game or event.
Empty Trash cans (on your side of field) after every game or event.
Report Any and All Safety Issues to Board Member and/or Safety Officer of Lincoln Park American Little League. If not readily available or on site, Phone Message, email, note in wing mailbox with contact information for follow-up or clarification of Issue.

Turn in Accurate Safety Report to Safety Officer in a timely matter. Follow-up with particpants Parent or Guardian when possible.

Keep current and accurate records of practice, game and event participation (Attendance)

Turn in (where applicable) Scores, Pitching Records, Ejection Records of Player, Coaching Staff, Parents and Spectators.
All Original Agreements are to have the Coach's "Wet Signature" applied.

These Contracts are "AT WILL", and may be cancelled with or without cause, and at anytime by the Coach, Lincoln Park American Little League in accordance with the rules, or by the member organization for which the coach is providing his or her services.

Lincoln Park American Little League Coaching Contracts are valid for one year from the date they are completed and signed.

* Sex offender Registry Checks
* Addresses
I the undersigned, authorize this information to be obtained either in writing or via telephone in connection with my volunteer application. Any person, firm or organization providing informationor records in accordance with this authorization is released from any and all claims of liability for compliance. Such information will be held in confidence in accordance with the organization's guidlines.




FAILURE TO COMPLETE THE ENTIRE AGREEMENT INCLUDING QUESTION, OR DELIBERATELY ANSWERING ANY QUESTIONS INCORRECTLY, WILL RESULT IN REJECTION AND/OR TERMINATION OF AGREEMENT.
