Continue Training During Summer Break With A Fun & Exciting Camp by Collier FC Surge Hosted by Our Director of Coaching ROB STOUTENBURG

Collier FC Surge Director of Coaching, Rob Stoutenburg, will be hosting a Soccer Summer Camp for boys and girls. The Camp will follow Pro Touch International Soccer Academy curriculum for player development focusing on ball control, agility, passing & trapping, tactical awareness, and shooting. Former USA Men's National Team player Cory Gibbs will make an appearance at the soccer camp.

There will be 2 sessions: June 15-19 and August 3-7, held 9am to 1pm daily. The cost will be $150 per session. Daily rates will also be available.

MEDICAL/EMERGENCY CONTACT INFORMATION

WAIVER AND RELEASE OF LIABILITY
In consideration of my child/ward being allowed to participate in any way in the Marco Island Soccer Club, Inc. youth sports program, related events and activities, the undersigned acknowledges, appreciates, and agrees that:

Although Club rules, equipment, training and discipline are designed to avoid any injury, the risk of injury from the activities involved in this program is significant, including the potential for permanent disability and death.
On behalf of my child and for myself, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, arising out of, related to and resulting from participation in this program, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE of the Marco Island Soccer Club their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and leasers of premises used to conduct the event "RELEASEES") or others, and assume full responsibility for my child/ward participation to the fullest extent of the law.

If I observe any risk of injury or death which is not inherent in the program while my child/ward participates in the program, I will remove my child/ward from participation and bring such to the attention of the nearest
official immediately; and, I, for myself and my child/ward on behalf of our heirs, assign personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS, the RELEASEES WITH RESPECT TO ANY AND ALL INJURY,DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASES OR OTHERWISE.

I hereby certify that, as the parent or guardian for my child, I have legal responsibility for and authority to sign this RELEASE AND WAIVER on behalf of my child. I further certify that I have read this RELEASE AND WAIVER in full, understand the same and have signed it voluntarily and with out any duress or coercion.

I understand that I am responsible for accident insurance coverage for my child.

I further grant the Released Parties the right to photograph and/or videotape me and/or my child or ward and further to display, use and/or otherwise use my and/or my child or ward's name, face, likeness, voice, and
appearance forever and throughout the world, in all media, whether now known or hereafter devised, throughout the universe in perpetuity (including, without limitation, in online webcasts, television, motion pictures, films, newspapers, and magazines) and in all forms including, without limitation, digitized images,
whether for advertising, publicity, or promotional purposes, including, without limitation, publication of Event results and standings, without compensation, reservation or limitation.

This Waiver and Permission for shall be governed by the laws of the State of Florida.
CODE OF CONDUCT POLICY
In an effort to promote good sportsmanship, the Marco Island Soccer Club has pledged to enforce a code of conduct that we hope will ensure the physical and emotional well being of all its participants. Our goal is to ensure that all of our participants will thrive in an environment where respect, team play, and camaraderie abound. Marco Island Soccer Club recognizes the physical and aggressive nature of the sport and, as such, we recognize the need for control. As parents and coaches, it is our responsibility to lead by example. Therefore, all parents and coaches are responsible to read and sign this Code of Conduct and to adhere by it. All participants who are deemed to be in violation of the Code of Conduct are subject to suspension and/or expulsion from the program. Parents who are found to be in violation may be barred from attending future games and/or forfeit their child’s right to participate in the program.

Such offenses that may warrant suspension or expulsion are as follows:
1) Abuse of a game official, coach, tournament officer, player, or spectator, whether it is verbal or physical, including but not limited to swearing, violence, obscene gestures, and other aggressive/inappropriate behavior reported to Marco Island Soccer Club.
2) Gross misconduct, including but not limited to fighting with an opposing player on or about the playing fields.
3) Intentional acts that could result in bodily injury or emotional harm to any person on or about the playing fields.

Coaches will be responsible for routine discipline of minor infractions as they incur within their own teams. Any infraction that cannot be controlled at the team level or that the coach believes warrants, will be referred to the League President or Board of Directors for further action. Coaches are to place the emotional and physical well being of their players ahead of their desire to win. Coaches will lead by example in demonstrating fair play and sportsmanship to their players. Swearing and excess physical contact will not be tolerated and complaints of such shall be referred to the League President or Board of Directors for further action.

Parents and other spectators are expected to express their enthusiasm positively during games. Any negative expressions (booing, jeering, etc.) will be considered in violation of this code. Furthermore, parents/spectators are not to approach coaches during or immediately after a game with questions or
complaints relative to playing time, positions, game strategy, etc. These matters should be taken up in a private conversation at a later (and less emotional) time.

Marco Island Soccer Club hopes to enhance the development of its participants. Our goals are to benefit the children who choose to participate. The above guidelines are set as a reminder that as parents and coaches we are to act as positive examples for our children to follow. With your help, these disciplinary
measures may never need to be imposed.

I have read and understand the Marco Island Soccer Club Code of Conduct and by checking the box below on behalf of myself, spouse, and other family members, agree to abide by its contents:
In submitting this registration, you are required to provide a copy of your child's birth certificate, passport or other proof of age, a completed copy of the Medical Release Form,and when required a Liability Waiver require by affiliated tournament sponsors. Your registration will not be completed until these documents are accepted by a Marco Island Soccer Club representative. Participation in the club will depend on availability of coaches and players for your given age bracket.
