2020 Manager Form


All SJMSBL Managers need to complete and sign the "2020 Manager Form." All 2020 Managers are responsible for and obligated to verify that their players and coaches completed, signed, and submitted (electronically or by paper copy) the Waiver Form. Paper copies should be emailed to eecnph@comcast.net or lmarshall@unfi.com.

BEFORE SIGNING, PLEASE REVIEW YOUR ROSTER TO VERIFY SUBMISSION OF THE WAIVER FORM BY ALL PLAYERS AND COACHES.




Terms of Acceptance and Signature


I submit this form to confirm that all players and coaches on my team have signed and submitted the Updated Waiver Form. By signing this document electronically, I understand that clicking the Electronic Signature checkbox and the Submit Form buttons constitute a legal signature, confirming that I acknowledge and agree to all of the stated conditions.


To SIGN ELECTRONICALLY and SUBMIT:

Step 1. Click the checkbox for "Electronic Signature." By checking this box and typing your name below, you are electronically signing your SJMSBL Manager Form.

Step 2. Type in your first name, last name, and today's date.

Step 3. Click the "Submit Form" button.

Step 4. On the next screen, you have the option to Edit, Print, or Cancel your Waiver Form.

Step 5. To officially submit your Manager Form, you must click the "CHECKOUT - Click Here to Complete Your Registration" button.

To submit the Manager Form electronically, fill in all the required fields (*), electronically sign the form, click the "Submit Form" button, and then click the "CHECKOUT " button on the next page.
