Team Registration Form

Submit this form to register your team for any of our adult sports leagues offered at the Spring Valley Gymnasium. A $100 deposit is due at the time of registration, and the remainder of the league fees are due by the second game of the season. Ref fees shall be paid each game directly to the officials. Please allow 24 hours for the Spring Valley Gymnasium to contact you to complete registration.
