2018 Wheeler Junior Football Registration Form

The Wheeler Junior Football program is entirely managed by volunteers including the coaches and Board of Directors. Our goal is to support the student athlete that wants to participate in football, provide an environment that will foster both their love and knowledge of the game, give them opportunity to compete, and help prepare them for varsity football. Our home games are played at Wheeler High School and our practice facilities are 1) Wheeler HS Field when available and 2) the field located behind East Cobb Middle School.

6th Grade Team can't turn 13 before August 1, 2018
7th Grade Team can't turn 14 before August 1, 2018
8th Grade Team can't turn 15 before May 1, 2018

Wheeler Junior Football is a volunteer organization and requires parent volunteers to successfully run the program. Each family is required to work at least one home event. Please tell us in which areas you would like to help:

2018 Wheeler Junior Football Refund Policy:
The goal of the Wheeler Junior Football Program is to support the student/player that wants to participate in football, provide an environment that will develop their appreciation and knowledge of the game, give them an opportunity to compete, and help prepare them for varsity football. However, we are a competitive football program and no student/player is guaranteed a minimum number of plays in a game situation. All players will participate in practice. Other leagues can be recommended if this is a concern of the parents or player.

The refund of registration fees is outlined below:

1. Should a player leave the team before the first practice following the player's registration, all of his registration fees shall be refunded, upon return of issued equipment.
2. Should a player registered to participate in Spring training leave the team after his first practice of Spring, but before the team's first practice of the Fall, one-half of the registration fee (not including the facilty use fee) shall be funded, upon return of issued equipment.
3. Should a player who did not participate in Spring training leave the team after his first practice for the Fall season, but before the first game of the Fall season, one-half of the registration fee (not including the facilty use fee) shall be funded, upon return of issued equipment.
4. Should a player leave the team after the first game of the Fall season, none of the registration fee shall be refunded.

Parent/Guardian Code of Conduct
All parents/guardians who have children participating within the Wheeler Junior Football program must abide by a Code of Conduct, which includes the provisions that follow. Any violation of these rules may result in immediate expulsion from the Wheeler Junior Football program. In addition, you will forfeit your registration for the current season and be subject for review for any other subsequent seasons.

All Parents/Guardians Agree To:

1. I/We will encourage good sportsmanship by demonstrating positive support for all players, coaches, officials and Wheeler Junior Football Board Members at every Wheeler Junior Football event.
2. I/We will place the emotional and physical well-being of my/our child ahead of my/our personal desire to win.
3. I/We will ask my/our child to treat all other players, coaches, fans and officials with respect regardless of race, sex, creed or ability.
4. I/We agree to treat all players, coaches, fans, officials, and Wheeler Junior Football Board Members with respect regardless of race, sex, creed or ability.
5. I/We will do my very best to make sure my child arrives and is picked up at the scheduled time for all practices, games and other Wheeler Junior Football events.
6. I/We agree to not smoke on the practice or playing field, or in the presence of a gathering of the team/squad at any Wheeler Junior Football game or event.
7. I/We agree to abstain from the possession and drinking of alcoholic beverages and the possession or use of any illegal substance on the practice or playing field, or in the presence of a gathering of the team/squad at any Wheeler Junior Football event.
8. I/We agree not to deliberately incite and/or participate in unsportsmanlike conduct at any Wheeler Junior Football game or event.
9. I/We agree not to use abusive or profane language or actions at any time at any Wheeler game or event.
10. I/We agree not to criticize, belittle, antagonize, and berate any player, coach, cheerleader, fan, official, parent, or Wheeler Junior Football Board Member by word of mouth or by gesture.
11. I/We agree to take responsibility for any actions that violates this Code of Conduct by a guest or relative of attending parent or guardian.
12. Any act of disrespect from I/We or a relative or guest may be cause for immediate ejection from any and all
current and future Wheeler Junior Football activities until reviewed by the Wheeler Junior Football Board of Directors.
13. I/We agree if I/We have been ejected or removed from any Wheeler Junior Football function I/We will refrain from attending any practices/games/events for the period of my punishment as decided by the Wheeler Junior Football Board of Directors.

I do hereby certify by my signature below as a parent/guardian of a child participating within Wheeler Junior Football that l/We have read and received a copy of the Parents/Guardians Code of Conduct and agree to abide by the terms and conditions set forth. I am also aware that Wheeler Junior Football has a ZERO TOLERANCE POLICY for violations of this Code of Conduct.
