**In addition to giving full consent for my child's participation, I understand and recognize the risk of physical injury inherent in cheerleading/tumbling and performances and I am willing to assume those risks. I agree that I will not hold Wylie Competitive Youth Football Association (WCYFA) or its directors, coaches, or instructors liable for injuries sustained while in attendance and/or participating in any WCYFA sponsored activity.

**In addition, I have read ALL the information in the Cheerleading Handbook and my child and I agree to abide by ALL rules. Parents/guardians or participants who do not abide by these rules may be restricted from WCYFA activities.

**No one will be fitted for a uniform if the first payment is not made on the date of the scheduled fitting. If payments are not current according to dates indicated in handbook, members will not be allowed to attend or perform at any functions and uniforms will not be distributed.

**I understand that if my child quits the program or is removed before the end of the season, no refunds will be given.

**Finally, I understand that my child must complete the season to receive end of season awards and recognition.
