Cheer Requirements

 

Listed below are the requirements needed for your Cheerleader. If you have any questions or concerns, you may reach out to Brian Beck (President) or Kristina Walden (Cheer Coordinator). Their contact info is listed under the Contact us tab.

 

 

 

1.       Registration Form

2.       Copy of Birth Certificate

  

·         If you have a returning cheerleader,  we already have a copy of the Birth Certificate on file.

 

3.       Medical Release Form

·         Must be filled out by a physician

 

·         If your child gets a yearly physical, your physician can fill the form out from that check up as long as it has been within the last 12 months.

 

·         This form must be filled out and turned in before you cheerleader can participate.

 

4.       Registration Fee

 

·         $80/ Cheerleader

 

·         Registration fee must be paid in full or Cheerleader will not be issued a uniform.

 

·         Registration fee covers uniforms and any equipment needed. You will need to provide Tennis Shoes.
