Good evening Minisink Youth Football and Cheer family.

 

This Sunday, April 28th, from 08:00 to 11:00 will be our first in person registration.  Registration will be held at theGreenville Volunteer Ambulance Building, address below:

 

1495 Greenville Turnpike, Port Jervis, NY 12771

 

To help move the registration process along please print out and complete the attached registration documents.  If you can have the Medical Release Form completed for Sunday, that is great, however it will not be required at this time to complete registration.  The other forms are required to get your child registered.

Also attached to this email is the 2019 Registration Flyer.  We will have football jerseys to try on at in person registration.  If you cannot make this in person registration, we will be hosting two other events, one on May 19th and the other on June 30th.  These will be held at the Greenville Senior Center on Route 6 from 08:00 to 12:00, address below:

 

 

Lastly, at in person registration we will be accepting payment via credit card, check or cash.
